Registration

If you wish to register and live within our practice area, please come into the surgery and fill in a registration form. You will also need to bring some photographic ID and proof of residency (for example a utility bill).

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. When you come to the surgery to sign your registration form you will be asked to complete the medical questionnaire.

We can provide our practice leaflet (and other information) in large print format if you are visually impaired. Please request these from reception.

PLEASE INFORM RECEPTION IF YOU HAVE MOVED ADDRESS OR CHANGED YOUR CONTACT NUMBERS.

First, download the national GMS1 form above  which is a national requirement or pick up some forms from the surgery.

Second, complete all of the forms.

Third, return it to the practice in person with scanned / photographed proof of identity and proof of address.
