New Patients

We welcome new patients who live within our practice area.

Park Road Surgery catchment area

You will be asked to complete a new patient health questionnaire when you register, covering your previous medical problems, family health problems and aspects of your lifestyle which affect your health. The Practice Nurses may ask you to come in to for a health check, for example to measure blood pressure, height and weight and any other areas of concern. Please bring a urine sample with you.

We can provide our practice leaflet (and other information) in large print format if you are visually impaired. Please request these from reception.

Please note that you will be allocated to a named GP; please ask at reception or at your next appointment if you would like to know who your named GP is. For on-the-day and emergency appointments, you will be asked to see any available doctor if your named GP is not available.

Please note that it is our practice policy to require patients moving out of our existing catchment area to register at a new local GP practice as soon as possible. We feel that your health care needs can be better met by having a local surgery who can utilise other local health services if required.

Please ensure that we have your up-to-date contact details, including mobile telephone number and email address where applicable.

We send out text reminders for pre-booked appointments and may also send texts or emails on occasion for a variety of purposes, for example to invite you to a vaccination clinic or health check, informing you about a change to the practice or asking you to update our records on your smoking status.

We are a Safe Surgery for everyone in our practice area:

  • Everyone living in England has the right to free care from a GP
  • Ask reception for an interpreter if you find it difficult to communicate in English
  • Our receptionists won’t ask about your immigration status
  • If you are worried about giving us your address, please let us know. Your information is safe with us
