2 – VOLUNTARY SECTOR & SOCIAL INITIATIVES
a) ABC – Active Breathing Course Referral Form (6 weeks long, 40min sessions weekly, virtual. For patients 12+ weeks post-Covid with ongoing low-level anxiety, breathlessness, disordered breathing. Based around singing.
b) Fulham FC (2 courses, 1 to run F2F at Kingsmeadow More than a Game – Mental Health Support, 1 to run virtually, we can refer both

3 – PUBLIC HEALTH & ADULT SOCIAL CARE KINGSTON
a) Get Active – a 12 week course including online 1:1’s with immediate effect for people experiencing post-Covid symptoms. We will also refer on to any community offers that will be of benefit to patients during the course

a) Offering guided self-help and CBT for Long COVID symptoms. Long COVID Group offering psychological support for low to moderate symptoms – due to run from May 21. Self-referrals via website or from health professional.
b) Long Covid Health Management Group – enabling the normalisation of low-severity ongoing symptoms (in development)

6 – IAPT – RICHMOND
a) Offering a stepped-care model of CBT-based therapy approaches and guided self-help (Please use standard referral form)
b) Covid workshop – for people worried about the Pandemic
c) Covid support group
d) Breathe-well group
e) ACT Group : Mindfullness-based stress-reduction
